Prescribing in practice Essay

Introduction The following assignment will discuss and analyse a prescribing episode, within practice, furthermore will outline safe prescribing from the Nurse Prescribing Formulary (NPF, 2013-2015).To be able to analyse and reflect on my new role as Community Practitioner Nurse Prescriber (CPNP) I will use Gibbs (1988) reflective model and a structure that will allow the use of a consultation model (Appendix1,Fig1). The focus on prescribing within nursing profession was first brought into discussion by Royal College of Nursing (RCN) in 1980 but has proven to be cornerstone after was part of the government agenda as a result of Cumberlege report in 1996.This report outlined the need for nurse to become a prescriber especially in community settings to provide clients with better care, safe and practical access to medication (Nuttal, 2008). Furthermore â€Å"The Medicinal Product Prescribing Act â€Å"1992 outlined changes in circumstances including nurses as prescribers, followed by recommendati ons made by Crown Reports1999 that suggest that health visitors (HV)) should be authorised to prescribe from a limited list, identified as the nurse prescriber’s formulary (NPF). (DH,2004). Scenario For the purpose of this assignment all names has been changed to maintain confidentiality according to NMC (2008).The following scenario it is in relation to a situation experienced while on the morning baby clinic working alongside my mentor .A mother Lisa come for a regular weight check for her 4 month old infant called James .While discussing with Lisa her son weight progress ,she mentioned about her son having very dry scaly skin on scalp since birth moreover Lisa explained that sometimes skin on the scalp become like a patchy crusty rash and James feel uncomfortable , at times not settling. Edwards (2010) suggest that using a holistic assessment it is very important when coming in contact with the client as the problem presented can have a drastic impact on the quality of life for child and the family. As a student under the supervision of my mentor we started a holistic assessment of James and finalise with a prescription decision. I start by using open questions related to Jam es general health and I asked  Lisa if she had any concerns but she confirmed back to me that there was no issues with his health since birth .To make sure that information provided by Lisa was correct and I have accuracy in details exposed I repeated back and she confirmed. As a student practitioner prescriber it is very important to maintain good practice and use assessment frameworks and consultation models as possibility to improve and gain better skills(Nuttall,2008). The Consultation The purpose of the consultation model in prescribing practice is to assist nurses in identifying strategies to deliver a diagnoses and ultimately the appropriate prescription .There are various consultation models used in practice with normative or descriptive character , however for the purpose of this case scenario I will concentrate on Roger Neighbour 1987 consultation model. Moreover Neighbour (2005) outline through his model consultation as a journey divided by check points , connecting which is the first point where client establish the relation with the practitioner is essential followed by summarising, handing over, safety netting and housekeeping. To progress further it is relevant to mention influence of communication skills in consultation process furthermore considering that nurse practitioners don’t need different communication skills in comparison with other professionals prescribers (While,2002) .However(While,2002)found that the need for appropriate environment that can maintain privacy and confidentiality could be a potential barrier during consultation rather than communication especial for those working in client homes. I choose Neighbour 1987 model for consistency and easy to apply in any circumstances moreover stand as a journey establishing relations with the client through empathy and in the same time identify the need for balance between practitioner prescriber and client (Tate,2010).Another positive aspect that I took in consideration when choosing the model was also housekeeping as practitioner have the opportunity through this section to become reflective to avoid any unresolved issues before consultation is complete ( Neighbour,2005) .Despite the decision made to use Neighbour 1987 model I will move further and explain the process and prescribing decision using the principles of good prescribing pyramid ,( Appendix 1, Fig 2)(NPC,1999). Step 1 Assessment; Consider the patient The beginning of consultation according to Neighbour,(1987) is connecting with Lisa and building a relationship ,however in a noisy environment like the clinic I found this difficult .Despite this barrier I manage to find a sitting area that appear to be more private and had the opportunity to manage a discussion with Lisa to establish that I needed to do a physical examination on James .As James was under 16 years of age ,consent from parent or guardian was needed ,therefore Lisa give a verbal consent as she was the biological parent (NMC,2013).To proceed further I use mnemonic OLDCARD to asses James with Lisa’s assistance trying to interact with her and find out if any physical or affective changes occurred. Bryans (2000) suggest that during assessment the practitioner should use knowledge, experience, recognition and prioritisation ,so to proceed further with a holistic assessment and identify symptoms that will help me to rule a diagnosis I felt I needed to ask Lisa more questions .Following the identified symptoms and physical examination I conclude that this was cradle cap a form of seborrheic dermatitis mainly affecting skin on the scalp with patches and thick scaling and sometimes yellow crusty (Sheffield et al,2007).It is important to rule out other conditions when taking assessments as sometimes cradle cap can turn into atopic dermatitis or fungal infection such as tinea capitis or easily misdiagnosed with crusted scabies (Yoshizumi and Harada,2008).Further continuing my assessment I was able to conclude that none of this conditions were present in James situation . It was important to discuss with Lisa during consultation family history that could provide additional support for my final diagnostic conclusion .Going thru such topic area Lisa explain that her husband James dad was suffering with atopic eczema since childhood .This was quite an important piece of information as such conditions like atopic eczema ( dermatitis ) are hereditary conditions often (National Eczema Society ,2011).Atopic dermatitis or eczema is a chronic skin disorder inflammatory with pruritic skin that appears mostly on the face ,neck ,bends of the arms or legs caused by the malfunction in the skin barrier( NICE,2013). Step 2 Which Strategy? In order to progress further following discussion with Lisa under my mentor supervision as a CPNP V100 I made a prescribing decision based on the physical examination and the information provided I concluded that James was suffering from cradle cup (seborrheic dermatitis)(NICE,2013).Furthermore Lisa confirmed that James did not have any allergies and was not on any medication .It was important to relieve the discomfort and unsettling times for James and I decided to prescribe an emollient and a bath additive. Pendleton et al (1984) suggest that practitioner should discover client expectation furthermore should take in consideration other treatment options before prescribing .According to NICE (2013) greasy emollients and soap substitutes or bath additive helps to remove effectively the scales, further more regular washing of the scalp and gentle brushing can help to loosen scales. Another stage followed in the Neighbour (1987) is handing over period but after Justin skin on his scalp was carefully examined no evidence of infection was identified so no referral to the General Practitioner (GP) was necessary. Consider the choice of Product According to NPC (1999) practitioners should use mnemonic ‘EASE’ as can be seen in the chart below to choose appropriate cost effective product for clients. E How effective is the product? A It is appropriate for the client? S How safe is it? E Is the prescription cost effective? To make the appropriate decision for Lisa’s infant, under the supervision of my mentor I used the Nurse Prescribers Formulary (NPF) and decided to  prescribe Oilatum Junior bath additive and cream as I considered this combination more effective in Justin’s treatment .Moreover the packaging was also appropriate as comes in a pump action and this could reduce the infection risks associated with emollients and types of recipients manufactured (NPF,2013-2015). When deciding the prescription products and quantities I also took in consideration Lisa requirements furthermore according to NICE guidelines emollients should be prescribe in large quantities approximately 250-500 grams per week and in the same time cost effective (NICE,2007).It is important when prescribing emollients to discuss with clients /parents /carers possible side effects even this products are considered to be quite safe ,however bath additives should be taken in consideration because tend to leave the s kin slippery after bathing the infant( BNFC,2014) .When prescribing a product especially for infants it is important to educate the parent/carer how to use the product and make sure they are aware of any risks and side effects . Negotiate a contract According to (Courtenay and Griffiths,2005) when prescribing we should view the process as a shared decision-making between client and prescriber .The prescribing decision stands as a contract so it was important to remind myself that I should take in consideration Lisa satisfaction with my decisions .To achieve all my actions it was paramount to have effective communication skills and ability to identify the appropriate therapeutic treatment .Throughout entire process I think that Lisa felt empowered as I hand it over to her the responsibility to apply the emollients on Justin skin ( Braid,2001).As I am not yet non-medical prescriber the prescription for Lisa’s son was written by my mentor and information leaflets regarding seborreic dermatitis were given to Lisa for further guidance. Review According to Neighbour (1987) consultation model this step relates to safety netting and refers to follow up of the consultation and possible outcomes. Furthermore together with my mentor we arranged to follow up in ten days to see if Lisa’s infant was making any progress and treatment was effective .Through this review in ten day time practitioner can identify any more concerns of the parents and possible side effects of the treatment used(  DH,2010b). Record Keeping Following guidance of the Nursing and Midwifery Council (NMC,2006) it is practitioner accountability to ensure records are maintain accurately and prescription details it is recorded in the infant health record (NPC,1999).Moreover I ensured the details of the prescription and the consultation were entered into general computer system within the next 4 hours following consultation and the GP also was aware of the consultation and products prescribed .According to (NMC,2006) details of the consultation and prescription should be entered into the computer system within forty eight hours with exception for special circumstances. Reflection Using Gibbs reflective cycle during consultation process with Lisa’s infant I had the opportunity to identify positive and negative aspects and reflect on the scenario. In the same time final part of Neighbours consultation model refers to Housekeeping and this stands as another stage that give me the opportunity to reflect on my prescribing decision and consultation .I felt that throughout the whole process I used safe and effective decisions even at times I felt a bit nervous however my mentor feedback was positive in regards to my performance .One aspect that I found difficult was the environment ,at the time the baby clinic was busy and noisy and it was difficult to find a private area to conduct the consultation ,but such issues I could reflect on it for my future prescribing practice . Conclusion Through this experience I able to develop on my new role as V100 prescriber using specific tools like the consultation model and good prescribing steps .Furthermore I had the opportunity to apply appropriate communication skills and medical knowledge along with the legislation to conclude with a safe and effective prescribing practice . A new CPNP I can utilize different assessment tools based on a consultation model along with a decision making framework to support my practice and at the same time reflect on my ability to undertake a partnership approach that responds to client needs and concerns. Motivational Leadership Essay

Effective leadership has a tremendous impact on the success of an organization. Darling & Heller (2011) infers that â€Å"with regard to individuals in leadership roles, their thoughts and feelings, fundamentally based on attitudes, thereby also control the levels of organizational success and every other aspect of an organizational leader’s arena of endeavor† (p. 9). Achieving optimal organizational performance is very much so dependent on the effectiveness of that organization’s leaders. Five topics interrelated to leadership effectiveness will be addressed in this paper. (1) Feedback works for leadership and tell why you think it could be beneficial. (2) Present an overview of the Contingency Theory of Leadership effectiveness to include Situational Leadership II. (3) Share some ways that you as a leader could lead your company in being socially responsible and creating an ethical organization. (4) Provide a brief analysis of empowerment; be certain to include the relationship between delegation and empowerment. (5) Discuss how leaders can foster teamwork within a group. All of these topics and/or concepts are essential aspects to achieving high levels of quality, roductivity and satisfaction within an organization. The concept of 360-degree Feedback is to formally evaluate an organization’s leaders through feedback from individuals that work closely with those leaders (e. g. subordinates, colleagues, direct supervisors, other superiors, suppliers and/or customers). According to Dubrin (2010), 360-degree Feedback is also referred to as multisource or multirater feedback, and is systematically derived from a full sampling of parties who directly interact with the leader (p. 110). One particular method for organizations to easure leadership effectiveness through the 360-degree Feedback concept is to set up a system utilizing the internet or the organization’s intranet. That system should be based on the attributes that organization deems to be the most significant and valuable. The 360-degree Feedback is of great benefit to leaders in providing them insight into the effectiveness of their behaviors and styles. The feedback mainly serves for developmental purposes (behaviors, attitudes, and styles), and not for the purposes of evaluating the leader’s performances. The 360-degree Feedback not only benefits the ndividual leaders, but it also is a vital component to the organization, as a whole. Organizations benefit from 360-degree feedback instrument in the following ways: (a) individual performance improvement, (b) impact on business goals, and (c) impact on organizational culture (Rehbine-Zentis, 2007, p. 160). The Contingency Theory of Leadership is the assumption that a leader’s ability to lead is contingently based on various situational factors, internally and externally. The following are various factors that could greatly influence a leader’s effectiveness within heir organization: the capabilities of the workforce, the culture within the organization, and the competiveness of market. Leaders who are have been highly effective within one organization, may not deliver that same effectiveness within a different organization. An important perspective on implementing contingency leadership is that the leader must be flexible enough to avoid adhering to old ideas that no longer fit the current circumstances or situation (Dubrin, 2010, p. 133). I can definitely see the validity of the theory in reference to my own experiences in previous leadership positions. It is absolutely about he ability to successfully and effectively adapt to different organizational environments. Like the Contingency Theory of Leadership, the Situational Leadership II style is a contingency leadership approach that is solely based on the theory that there is no one particular best style of leadership that fits all situations. This leadership style focuses mainly on the capabilities or behaviors of individuals and/or groups that one leads. Blanchard (1997) asserts, â€Å"Situational Leadership II advocates that the best leaders provide the amount and kind of direction and support which best fits the developmental evel of the employee† (p. 38). As a leader, I can encourage my organization to focus on ethical business practices and social responsibility, beyond profit pursuit, based upon our strategic organizational goals, mission and vision statements. I would initially ensure that the organization’s environment was a positive, engaging culture conducive to intellectual development. Individuals within an organization invest about one-third of their time at work, a pleasant work environment increases the chances that their life will be enriched (Dubrin, 2010, p. 181). There would be a significance placed on competing fairly and onestly, not causing harm to others (internally/externally), and communicating truthfully. I would ensure that my organization is offering goods and services that are valuable and essential to consumers. We would be consistent in providing quality employment to individuals which would benefit them in providing for their families, in the forms of, paying them salaries/wages, offering quality health insurance and/or child care services, matching retirement plan contributions, etc. We would also benefit society in maintaining ethical practices in our financial obligations through paying taxes which elps fund education, support government, build highways, as well as provide funds to enhance our local, state and national public safety/defense systems. According to Forsyth (1997), â€Å"genuine changes to business practices have to be made in order to protect the environment† (p. 272). I would ensure that, as an organization, we were utilizing the most up-to-date technology and economic solutions to reduce or alleviate any environmental issues that we faced. My organization would take every possible precaution to not generate unnecessary waste and pollution through the improper onsumption of resources, as well as from the possible emission of toxic air pollutants caused by our manufacturing processes. The concept of empowerment involves a power transfer from higher levels of employees to lower levels of employees within an organization (Khalid & Sarwar, 2011). Leaders that utilize positive empowering practices with their employees are more likely to increase their organization’s productivity. When employees are given the opportunity to share in the responsibilities of their leaders and/or managers, they feel more powerful and valued. Empowerment is also advantageous to an organization, in respect to owering labor costs through the sharing of those managerial responsibilities with its lower-level employees. According to Dubrin (2010), leaders can promote empowerment within their organization through fostering initiatives and employee responsibility, linking work activities to the organization’s strategic goals, providing ample information to their employees, and by allowing group member or individual employees to choose their preferred method for meeting a goal (p. 205-206). One contributing factor to empowerment is the act of delegation which is more of a formal ssignment of tasks and responsibilities. Delegation, as well as, empowerment can be a very motivational tool which offers individuals the benefit of skill development. The bottom line is that both concepts represent the core principle of accountability. Teamwork is vital to an organization’s success. Being that teams consist of individuals with different backgrounds, beliefs, ideas and points of view, it is imperative that we always be respectful and embrace the rewards that diversity brings within the team. As leaders, we can foster teamwork within a group through providing team embers with clear descriptions of the team’s goals, mission and tasks, commanding performance commitment and excellence, providing direction and feedback, alleviating micromanagement practices, forming virtual teams utilizing e-leadership, and encouraging power sharing which ensures that each team member is being heard, taken seriously and treated respectfully (Dubrin, 2010). More importantly, encouraging teamwork and utilizing teams are essential in increasing an organization’s productivity and achieving cohesiveness throughout the organization. A leader’s ability to build ffective teams committed to organizational goals tops the list of priorities in todays dynamic business landscape (â€Å"Work at fostering teamwork on the job,† 2011). Teams can produce higher-quality solutions, as well as foster creativity and positive energy. Effective teams are capable of being more adept at solving difficult, complex problems than that of the organization’s â€Å"top-performing† individuals. Creating teams can also cause an increase in commitment, lower stress levels, diminish internal-competiveness, and promote flexibility in responding to the organization’s needs, in relation to, â€Å"business† changing.

The study in discussion would like to help the understanding of this constructive, rhetorical view of reading.Throughout the article, Haas and Flower emphasize how students â€Å"are good readers in the traditional sense†¦ yet, they paraphrase rather than analyze, summarize rather than criticize texts† (170). This brings up two key points that Haas and Flower discuss in a relation to rhetorical reading strategies, in student reader-writers. According to the research Haas and Flower conducted, only about one percent of students use the rhetorical reading strategy, which means that most students don’t criticize and analyze text that they read.Most students mainly use the Content and Feature/F unctional reading strategy, rather than deconstructing the text, they just say the basic things, and summarize which brings up the question, how does the constructive process play itself out in the actual thinking process of reading? Haas and Flower purpose that readers â€Å"construct meaning by building multifaceted, interwoven, representation of knowledge† (168).The main focus Haas and Flower have throughout the article is the fact that students are staying at the â€Å"mediocre† level while reading, instead of breaking down the text and actually analyzing what the author(s) were trying to say. International educational opportunities are also increasing the availability of multi-cultural man power. The diverse understanding of business procedures and varied interpretation of management scenarios by this multi-cultural workforce adds complex cultural diversity to the work environment. Inter personal relationship The modern work groups hold much diversity in their inter-personal relationship attitudes. Although this may vary from person to person, the range of the diversity is much high with reference to the modern work groups which includes staff from different countries and cultures. Christopher & Diana (1996, p75) have mentioned seven factors concerning to the human relationships including time, space, odors, frankness, intimacy of relationships, values and expression of emotions. When an employee or a group from a particular part would insist on punctuality, others from different part of the globe would not pay much importance for time. Space is also viewed differently by different people. When some need to be close to their colleagues some others would prefer to keep a distance. Rollin Fitness Center should be opened at 8 in the morning and closed at 6 in the evening which amounts to 10 hours per day. Therefore; ?18/10 Hourly cost = ?1.8 per person This is the best way to put the charges since most people like spending few hours in the gym. Besides, since the Olympic Games are around the corner, most of the participants in the Olympic Games will only afford to spend few hours in the gym and that is the best cost for them (Kirk, 2000). The streams of ions are then separated based on their charge-to-mass ratio (m/z). Final step in mass spectroscopy involves measuring the current when these ions hit appropriate transducers. Major components of a mass spectrometer are an inlet system for injecting micro amounts of the sample to the ion source. Conversion of the sample into ions is achieved by bombarding the sample with electrons, ions or molecules. Upon samples being converted into ions, the stream of ions is directed into a mass analyzer which disperses the ions based on m/z ratio. From the mass analyzer, the transducer converts the beam of ions into electrical signal which are processed to generate a mass spectrum of the analyte. Method for ionization falls into two categories; the gas phase and desorption methods. Gas phase involves the vaporization and ionization of a sample whereas in desorption method; solid or liquid sample is converted into a gaseous phase directly. Matrix-assisted desorpti on-ionization (MALDI) and electrospray ionization (ESI) are examples of desorption ion sources for mass spectrometry and bear the advantage of being applicable to thermally unstable samples as well as in non-volatile samples. Electrospray ionization(ESI) is based on extraction of ions from solution (Rosenberg 845). ESI is amenable in the analyses of biomolecules such as polypeptides, proteins and oligonucleotides which have high molecular weight (100, 000 kDa) (Skoog pp560). Multiple charged ions are produced in ESI which enable measurement of high mass species. This is achieved by reducing m/z ratio to a range appropriate for mass spectrometry application. Samples used in ESI are usually non-volatile and the method is amenable for mass determination of biomolecules, analyzing and sequencing of proteins as well as analyzing drugs. There are many disputes to whether it was for the better or worse that the English had arrived in Australia and taken over. But the case still stands why the aboriginals had to tolerate the abuse, massacres and imported disease along with the loss of rights just because they were not as civilized. The Aboriginals were the first people to live in this country for over 45,000 years. Before the English had arrived they had an approximately a population of about 750,000. There is no excuse to the way they were treated but sadly they were not the only ones who were miss treated. Other indigenous communities around the world such as the American Indians were facing similar problems. The Aboriginals were facing the same problems everyday with the English. They were constantly being physically and mentally abused. Looked up for no reason in particular, no food and shelter and just getting pushed around all day. Some disputes would have been because of language, land and rights. The English would have the upper hand because of weaponry and technology. When the English came they brought with them alcohol, which later had a detrimental effect on the Aboriginals as there was a lot of abuse within the communities. The Native Americans were enslaved and forced to work in mines. This resulted in women killing their new born babies, men jumping off cliffs or consuming poisons. The European colonization had had a massive affect on the Native American population. Conflicts arose regularly between the Native Americans and the Europeans. Many people were imprisoned and abused during this time. The Aboriginals population had decreased dramatically due to massacres. These massacres may not have been intentionally for a specific reason but much of the population had decreased from this. One of the well known massacres to occur in the indigenous time happened at the battle mountain. An estimate of 200 indigenous people died during that killing and there were many more. The Native Americans had encountered major population downfall from the 1500 through to the 1890s due to massacres. On May 26th 1637, in mystic river side, English colonists had set fire and burned 600-700 civilians alive. This was one of the largest killing sprees during this time. This was just on example of how brutal the English were to the natives. When the white settlement came to Australia not only did they kill the Aboriginals with abuse, massacres and miss treatment but as they came they brought along diseases as well. These diseases included small pox, chicken pox, influenza and the measles. The Aboriginals died from such diseases because they were not immune to them and their traditional medicines did not help relieve the symptoms. So even such things as colds and flues could have swept through the nation and killed half the tribe. The Native Americans were introduced to such diseases as small pox, typhus, measles, influenza, bubonic plague, mumps and yellow fever. The natives would use sweat baths as a medical treatment which could have weakened the body and made them more immune to the disease. Not only did the Europeans bring over the diseases but as they imported animals that also carried diseases as well. These diseases were the major cause of death during the Native American era. As the first fleet arrived, the Aboriginals were losing their land. The more they stayed the fewer rights they had and eventually decreased to a minimal number of full breeds. The Aboriginals were forced to speak English and as a result of this the languages or dialects had died out. There were pubs built on native ancestral land which only white people could enter. They were forced to dress as the English and act as the English. Their rights had become nothing to be even thought. The Europeans treated the Aboriginal race as animals. Their land was cleared of natural vegetation to make way for buildings, house and roads. They were not allowed to even use them after they were built. As the Europeans entered America they miss treated the natives improperly. Slaves were put into mines or on land and forced to work. As new people came into the land the old people were relocated and forced to change their way of life. This slowly reduced birthrates therefore slowly reducing the population. The European settlement was much alike as the English in Australia. They gave the natives minimal rights or nor rights at all. They massacred tribes and destroyed ancient ground. They brought in the natives and enslaved them, made them speak differently and act differently thus destroying the population and race. Diseases were brought in from over seas either by human or animal and just the way they treated the indigenous people was unacceptable. But unfortunately they were not the only two facing the same problems. There were many more with bigger problems and some were irreversible. What the colonists needed to do is perhaps make friends with the indigenous people and respect their rights, land and race but still build a better future.
